Vicarage Forms
These downloadable forms are provided for current Supervisors and Students when submitting their vicarage and intern reports. These forms were designed to be filled out using a computer and Microsoft Word. For those supervisors of M.Div. students who will be filling out the forms in longhand or on a typewriter, the second grouping contains an alternate set of forms. Upon completion, these forms should be emailed (or mailed) to the Vicarage office.
In addition, the the current Evangelism Module guidelines for supervisors and students are provided below. These are the seminary's guidelines to assist with the major evangelism project required of every traditional M.Div vicar and Deaconess intern during the vicarage year.
Please be sure to choose the correct reporting form, as each program has a different report.
M.DIV DOWNLOADABLE FORMS:
FIRST and FINAL reporting forms for supervisors of traditional, alternate route and deferred M.Div. students only. (Forms for EIIT, DELTO, DTEP, SMP and Deaconess Intern students are located below).
SUBMITTING HANDWRITTEN FORMS:
Alternate forms for supervisors of traditional M.Div students who want to submit handwritten reports: FIRST and FINAL reporting forms for supervisors of traditional M.Div students.
DEACONESS INTERNS / DISTANCE EDUCATION DOWNLOADABLE FORMS:
Preliminary reporting forms for Supervisors of Deaconess, EIIT, DTEP, DELTO and SMP students (DTEP, DELTO & SMP are to use Distance Education reports (DE):
FINAL reporting forms for Supervisors of Deaconess, EIIT, DTEP, DELTO & SMP students. Again, DTEP, DELTO & SMP are to use the Distance Education (DE) report.
